Q: What should I bring to my appointment?

A: Please bring the following to your appointment:

  • Photo ID
  • Insurance card(s)
  • Co-payment, if needed
  • Current list of all medications, including vitamins and supplements, or actual prescription bottles

Q: What is you appointment cancellation policy?

A: Both of our ENT offices make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our office with 24 hours’ notice if you need to cancel the appointment.

Q: What are your billing requirements?

A: Patients must pay any co-payments at the time of their visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance, that their insurance company designates is their responsibility.

Q: Do you have financial assistance plans?

A: OhioHealth and our practice are pleased to offer financial assistance to patients with limited resources and inadequate medical insurance coverage. To be considered, you must provide copies of your income and complete an application.
