Here's some important information to help you prepare for your visit.
- New Patient: If you are a new patient, please complete this Patient Chart Summary Intake Form for Adults or Patient Chart Summary Intake Form for Children and bring it with you to your first visit.
- Release of Medical Records: If you need to transfer your medical records to/from another medical practice to/from OhioHealth Ear, Nose & Throat Physicians, please complete the Authorization for Use or Disclosure of Protected Health Information (PHI) Form and give it to the medical practice that currently has your records.
What to Bring to Your Appointment
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- Co-payment, if needed
- Current list of all medications, including vitamins and supplements, or actual prescription bottles
Appointment Cancellation Policy
We make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our office with 24 hours’ notice if you need to cancel the appointment.
Referrals from Another Physician
In most cases, we do request a referral from another physician before scheduling an appointment with our practice; however, don’t hesitate to call us to discuss your healthcare need. We can work with your primary care physician for a referral, if needed. If your other physician does refer you to us, we will contact you within three days of receiving the referral to set up an appointment for you.
Prescription Refill Policy
First, please check with your pharmacy to see if you have additional refills remaining on your prescriptions. If you believe your prescription will run out before your next office visit, you may call our office to discuss your need for a refill. If you are due for a follow-up appointment before a refill, we will schedule your next appointment. Prescriptions will not be refilled after office hours or on weekends.
After-Hours Medical Care
If our office is closed and your medical need is urgent, please call us at (419) 756.5500 to speak to the after-hours answering service. Your need will be evaluated, and you will be given direction on the proper medical care. If you have a life-threatening emergency, please call 9-1-1 or go to the hospital emergency department.
