Here's some important information to help you prepare for your visit.
- New Patient: If you are a new patient, please complete this Patient Chart Summary Intake Form for Adults or Patient Chart Summary Intake Form for Children and bring it with you to your first visit.
- Release of Medical Records: If you need to transfer your medical records to or from another medical practice to or from OhioHealth Ear, Nose & Throat Physicians, please complete the Authorization for Use or Disclosure of Protected Health Information (PHI) Form and give it to the medical practice that currently has your records.
What to Bring to Your Appointment
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- Co-payment, if needed
- Current list of all medications, including vitamins and supplements, or actual prescription bottles
Appointment Cancellation Policy
We make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our office with 24 hours’ notice if you need to cancel the appointment.
First, please check with your pharmacy to see if you have additional refills remaining on your prescriptions. If you believe your prescription will run out before your next office visit, you may request a refill by calling our office. If you are due for a follow-up appointment before a refill, we will call you to schedule. Prescriptions will not be refilled after office hours or on weekends.
After-Hours Medical Care
If our office is closed and your medical need is urgent, please go to urgent care or the hospital emergency department.
A referral from another physician is not needed to be seen by our ENT physician unless you have Medicaid insurance. If your primary care physician wishes to refer you to us, ask your physician’s office to call us, or they may complete our referral form and fax it to our office.
